The program comprises 4 classes (2 hours each), conducted online twice weekly for 2 weeks (live and recorded) in case you missed any class. The course blueprint will cover:
1. Ethical, Legal, and Professional Responsibilities of a Care Plan Pharmacist (CPP)
- Regulatory Knowledge: Understand Ontario and federal pharmacy laws, standards, and ethical codes.
- Ethical Practice: Apply ethical principles, manage conflicts of interest, and protect patient confidentiality.
- Professionalism: Maintain professionalism, engage in lifelong learning, and work collaboratively in healthcare teams.
2. Development and Implementation of an Individual Patient Care Plan
- Assessment: Conduct thorough patient evaluations and identify drug-related issues.
- Plan Development: Create SMART goals and integrate personalized therapeutic strategies, including non-drug interventions.
- Monitoring: Set up follow-ups, review outcomes, and update care plans as needed.
- Communication: Provide clear patient education and use motivational techniques to enhance adherence.
3. Proper Knowledge and Use of Research
- Evidence Evaluation: Critically assess the quality and applicability of research studies.
- Application: Integrate evidence-based guidelines with clinical expertise and patient needs.
- Research Literacy: Use key databases and understand basic statistical concepts.
- Innovation: Stay updated on new therapies and participate in quality improvement initiatives
